Types of Records Retrieved

•   Business, personnel and payroll - records may be obtained either by authorization or by subpoena depending on each company's policy. Send us your request and we will do the research for you.

•   Income tax – In order to obtain these records Form 4506 must be submitted. You can obtain four years of records with each authorization.

•   Medical, billing and radiology – These are the most commonly requested records and our staff is trained to obtain this sensitive information whether by subpoena or authorization. HIPAA has changed the way these records are released and many custodians will not release any records without a HIPAA compliant authorization. When an authorization is not available, a statement of assurance will be prepared and sent to your office.

•   Police, Sheriff, etc. - for the most part no authorization is needed. Just send us all pertinent information relating to the records you are seeking; for instance the date of the incident, location of the incident, and the names of the parties involved.

•   Social Security – In order to expedite this type of request please submit a Consent for Release of Information, signed by the subject of the request, the SSA does not respond to subpoenas.

•   Texas Workers Compensation Commission– For these requests Form 153 signed by the subject of the request, along with a copy of the Original Petition and Defendant's Answer. Send us all documents and we will submit accordingly and monitor the status of the request.
